We are seeking a compassionate and motivated Lead Case Manager to guide our Case Management Team in supporting pregnant and parenting teens and their children. This role leads two case managers, builds relationships with young parents, assesses needs, and connects families with supportive resources. Working closely with the Program Director and community partners, this position helps ensure teens and their children receive coordinated care and encouragement to thrive.
